Supreme Forms

New Member/Form 100

  • Form 100 (fillable)
  • Supreme Department: Fraternal Services
  • Video by Andy Airriess, PSD on YouTube explains how submit the Form 100 electronically (as a PDF) from your smartphone.
  • Officers Online Options:
    • You can use the PROSPECT Tab for an online member to join your council.
    • The CANDIDATE Tab can be used with the information from the Form 100.  See this FAQ about using the candidate tab.
  • Tips:
    • Combining multiple Form 100s in a single PDF is preferable, as long as all the Form 100s in a single PDF are of the same type (New Member are one type, Transfers are another). If you have different types of Form 100s, they should be submitted in different emails. Follow the instructions in the video (linked above).
    • Submitting as a JPG or other graphics file is not recommended.
    • If sending electronically, forms should not be sent using other means as well.
    • Not submitting it electronically?  Mail the original Form 100 to: 1 Columbus Plaza, New Haven, CT. 06510-3326 Att. Fraternal Services.

State Forms

State Service Program Award Entry – One copy of this form is to be used for each program your council enters for the State Service Program Awards (up to four, one for each of the Faith in Action program categories).  Attach extra pages as necessary. Create a PDF of each entry and submit each separately to the State Deputy by Midnight, March 15th. You can mail the forms (remembering to include 13 paper copies — the number of districts plus two) or email the forms once (as a PDF to Utah.State.Deputy.Forms@ gmail.com).  www.kofc.org/forms also has the Family of the Year form.  The “Knight of the Year” award submissions doesn’t have a form; it’s submitted on “council letterhead” and include similar information as the Family of the Year.

Chaplain of the Year submissions should be submitted using the “State Blessed Michael McGivney Award – Council Nominee ” form  SBMMA.  This form was introduced for the 2022-2023 fraternal year.

Utah State Member Conservation Program – This process provides the Council requirements, who can be suspended, and Council process including the Conservation Form. The Conservation Form must be completed prior to submission to the State “Membership Retention Chairman” Bill Kelly.

Forms to Help Run Your Council – This spreadsheet contains a good portion of all the forms you will need to be a strong and successful Knights council.

Subject: Join Us for our Knights of Columbus Mass with Fr. Cummins!
Brothers,
I hope this message finds you well and ready for a blessed opportunity to come together in faith and fellowship!
This month, we are thrilled to invite you our 2nd Mass celebrated by Fr. Cummins, said for the Knights, families and friends of Council 9849. It's going to be an evening of spiritual enrichment and community spirit at the Newman Center at Weber State University. Mark your calendars for Thursday, September 19st at 6:30 pm!
In the spirit of our faith's rich tradition, we'll be diving deep into both the Old and New Testament. Just as the Old Testament stories of valor and wisdom illuminate the path for us, so too does the New Testament bring us the teachings and grace of Jesus Christ. It’s a beautiful reminder of how our faith journey connects both the past and the present.
This Mass will be a wonderful opportunity to gather, reflect, and celebrate our shared mission as Knights. Fr. Cummins is eager to share a message that will resonate with both the ancient wisdom of the Old Testament and the transformative power of the New Testament.
So, come join us for an evening of prayer, reflection, and community. Bring your family, friends, and fellow Knights. Together, let's make this a memorable occasion for all!
We look forward to seeing you there!
Vivat Jesus!
George

Keep in mind, we will be having our Kick-Off Prayer Rally on Tuesday September 24 @ 5:30PM outside Planned Parenthood Metro (160 S. 1000 East). Come and be with others for prayer and receive materials, yard signs, and devotional pamphlets to pass out to others at your church.
I have put out the 40 DFL Flyer on the web page (40DaysforLife.com/SaltLakeCity). Please on the right side, click down under resources to find the PDF to help promote this years Peaceful Vigil.
Help to inspire others to pray at the Planned Parenthood Metro (160 S. 1000 East) to be a sign of hope for someone to consider LIFE and not death. They can sign up here.
I and others will be at PPM on Wednesdays at 11:00AM. If you need materials and support, come join us.
See you at the Kick-Off Prayer Rally - Tuesday, September 24 @ 5:30PM.
