  • Officers Online Options:
    • You can use the PROSPECT Tab for an online member to join your council.
    • The CANDIDATE Tab can be used with the information from the Form 100.  See this FAQ about using the candidate tab.
  • Tips:
    • Combining multiple Form 100s in a single PDF is preferable, as long as all the Form 100s in a single PDF are of the same type (New Member are one type, Transfers are another). If you have different types of Form 100s, they should be submitted in different emails. Follow the instructions in the video (linked above).
    • Submitting as a JPG or other graphics file is not recommended.
    • If sending electronically, forms should not be sent using other means as well.
    • Not submitting it electronically?  Mail the original Form 100 to: 1 Columbus Plaza, New Haven, CT. 06510-3326 Att. Fraternal Services.

State Forms

State Service Program Award Entry – One copy of this form is to be used for each program your council enters for the State Service Program Awards (up to four, one for each of the Faith in Action program categories).  Attach extra pages as necessary. Create a PDF of each entry and submit each separately to the State Deputy by Midnight, March 15th. You can mail the forms (remembering to include 13 paper copies — the number of districts plus two) or email the forms once (as a PDF to Utah.State.Deputy.Forms@ gmail.com).  www.kofc.org/forms also has the Family of the Year form.  The “Knight of the Year” award submissions doesn’t have a form; it’s submitted on “council letterhead” and include similar information as the Family of the Year.

Chaplain of the Year submissions should be submitted using the “State Blessed Michael McGivney Award – Council Nominee ” form  SBMMA.  This form was introduced for the 2022-2023 fraternal year.

Utah State Member Conservation Program – This process provides the Council requirements, who can be suspended, and Council process including the Conservation Form. The Conservation Form must be completed prior to submission to the State “Membership Retention Chairman” Bill Kelly.

Hello Fellow Prayer Warriors,
Welcome into a New Year of hope and work. Yes, I mean hope in what our prayers can do to make a difference to change hearts toward LIFE. And, I mean work in doing a Corporal AND Spiritual Work of Mercy in being active in your communities.
Special Event, we will have a Kick-Off Prayer Rally on Tuesday, February 13 @ 5:00 PM at Planned Parenthood Metro (160 S. 1000 East).
We have 40 days until the 2024 Spring Campaign begins on February 14 and ends March 24. On our 40DFL/SaltLakeCity page, you can find the 2024 Spring Flyer and Bulletin Announcement. Please feel free to copy and distribute these materials to all your churches, friends, family to get more people involved in saving the unborn and supporting families.

People can get involved by:
Prayer & Fasting - Pray at home, at your church, or in your car or on a walk.
Peaceful Vigils - Sign up for just one hour a week at Planned Parenthood Metro. Be a witness for LIFE in our society.
Community Outreach - get a bumper sticker or yard sign to show your support for LIFE.

Brother Knights,
Just a reminder that this month’s business meeting is being replaced by the Spaghetti dinner/St. Joseph’s Table this Tuesday at 6:30 at St. Thomas Aquinas’s Social Hall. Everyone from the Parish is welcome to attend.
At the beginning of the dinner, members of the State Council will be providing several checks to the planned pregnancy group for the ultrasound machine and some extra items like a large screen TV to view the ultrasounds. This is another reason to attend the dinner.
Family Director, Brother Matt Gordon is leading the effort for the Council. As previously mentioned, he is looking for volunteers to help with setup starting at about 5:00, serving and cleanup. Food has been taken care of, but if you want to bring something it will not be turned away,
Whether you can help with the dinner or can come to participate in helping eat the spaghetti, it would be great to see all of you and your families there to support this effort.

Everyone is welcome to come and learn more about the life of Jesus and enjoy the fellowship.
