New Member Form 100

  • Officers Online Options:
    • You can use the PROSPECT Tab for an online member to join your council.
    • The CANDIDATE Tab can be used with the information from the Form 100.  See this FAQ about using the candidate tab.
  • Tips:
    • Combining multiple Form 100s in a single PDF is preferable, as long as all the Form 100s in a single PDF are of the same type (New Member are one type, Transfers are another). If you have different types of Form 100s, they should be submitted in different emails. Follow the instructions in the video (linked above).
    • Submitting as a JPG or other graphics file is not recommended.
    • If sending electronically, forms should not be sent using other means as well.
    • Not submitting it electronically?  Mail the original Form 100 to: 1 Columbus Plaza, New Haven, CT. 06510-3326 Att. Fraternal Services.

State Forms

State Service Program Award Entry – One copy of this form is to be used for each program your council enters for the State Service Program Awards (up to four, one for each of the Faith in Action program categories). Attach extra pages as necessary. Create a PDF of each entry and submit each separately to the State Deputy by Midnight, March 15th. You can mail the forms (remembering to include 13 paper copies — the number of districts plus two) or email the forms once (as a PDF to Utah.State.Deputy.Forms — gmail.com).

Family of the Year form

The “Knight of the Year” Award submissions does not have a form; it’s submitted on “council letterhead” and include similar information as the Family of the Year.

Chaplain of the Year submissions should be submitted using the “State Blessed Michael McGivney Award – Council Nominee” form. This form was introduced for the 2022-2023 fraternal year.

Utah State Member Conservation Program – This process provides the Council requirements, who can be suspended, and Council process including the Conservation Form. The Conservation Form must be completed prior to submission to the State Membership Retention Chairman Bill Kelly.

Knowing your expertise and strong connection with the Knights of Columbus, I wanted to reach out with an invitation. St. Ann’s is hosting a Trunk or Treat for the local community on Saturday, October 25th, from 2:00–5:00 p.m. Our hope is to encourage families to attend Mass at the conclusion of the event, creating a natural bridge between community celebration and worship.

We are hoping to find sponsorship within the parish for a few opportunities:

  • Sponsoring a decorated car from the Knights. This would allow your group to participate directly in the Trunk or Treat, handing out candy to children from a Knights-sponsored car.

  • Donating a silent auction basket. Proceeds would go directly toward supporting students at KSA.

  • If being present is not possible, would you be open to using your network to help us find avenues to provide food or candy for the event?

I truly believe this would be mutually beneficial for both the school and the Knights. As you know, part of the mission of Catholic schools is to provide opportunities for family faith formation, including parents. This partnership could be a natural way to invite school parents into fellowship with the Knights of Columbus. (We are also working with the Women’s Guild to ensure mothers are offered similar opportunities.)
